<h2>What is the purpose of the Government’s Chronic Disease Co‑care Scheme?</h2>The Chronic Disease Co‑care Scheme (“The CDCC Scheme”) implements the concept of “Family doctor for all.” Through a district‑based, cross‑professional public‑private collaboration model coordinated by District Health Centers (DHCs) and DHC Express, the Scheme subsidizes the diagnosis and management of common chronic diseases in Hong Kong—specifically diabetes and hypertension—at private clinics.&nbsp;The Scheme emphasizes early detection and early treatment, as well as encouraging participants to practice self‑management in partnership with healthcare providers, achieving the goal of “co‑care.” The aim is to help patients manage chronic diseases more effectively and reduce the public’s demand for specialist and hospital services.&nbsp;Starting 7 February 2026, the service scope will be expanded to include the Hepatitis B Co‑care Program, which provides risk‑based screening and management for hepatitis B. Higher‑risk individuals can receive hepatitis B screening and personalized health management plans. The objective is to identify people with chronic hepatitis B in the community early and provide long‑term follow‑up to reduce risks of severe complications such as liver cirrhosis and liver cancer.&nbsp;<h2>Who are the main target groups of the Scheme?</h2>&nbsp;Chronic Disease Co‑care SchemeThe main target groups are individuals newly diagnosed with diabetes and/or hypertension.&nbsp;&nbsp; The Scheme does not cover existing diabetes/hypertension patients already receiving care in the public healthcare system.&nbsp;Hepatitis B Co‑care ProgramThe main target groups are individuals at higher risk of hepatitis B.&nbsp;<h2>Who is eligible to join the Scheme?</h2>&nbsp;Chronic Disease Co‑care Scheme<ul><li>Hong Kong residents aged 45 or above</li><li>With no known history of diabetes or hypertension</li><li>Willing to enroll in the Electronic Health Record Sharing System (eHRSS)</li></ul>&nbsp;Hepatitis B Co‑care ProgramHong Kong residents who:<ul><li>Were born in 1988 or earlier (the year the universal neonatal hepatitis B vaccination program started), and</li><li>Have a family member (parents, siblings, children) or sexual partner with chronic hepatitis B, and</li><li>Have never been diagnosed with chronic hepatitis B and shown no related symptoms, and</li><li>Have never completed the full hepatitis B vaccination course</li><li>Agree to enroll in the Electronic Health Record Sharing System (eHRSS)</li></ul>&nbsp;<h2>How to enroll and pair with a Quality HealthCare doctor?</h2>&nbsp;For Chronic Disease Co‑care SchemeIf you wish to join the Scheme and pair with a Quality HealthCare doctor:<ul><li>You may use the search engine on the Chronic Disease Co‑care Scheme website contact our doctor with the <img src="/uploads/ccbc_en_66d908c5e8.png" alt="Accept Scheme enrolment at clinic label">label, and contact them to enroll, pair up a doctor and schedule your first consultation directly.</li></ul>Alternatively:&nbsp;<ul><li>Visit any DHC or DHC Express for registration.&nbsp;&nbsp; Staff will provide a list of participating private doctors for you to choose from.&nbsp;&nbsp; After successful pairing, you will be arranged to receive screening and management services from your chosen doctor.</li></ul>&nbsp;Note: You may only be registered under one doctor at any given time.&nbsp;<h2>For Hepatitis B Co‑care Program</h2>If you are interested in joining the Hepatitis B Co‑care Program and being matched with a Quality HealthCare doctor as your designated doctor, you may first enquire at your nearby District Health Centre (DHC)/ District Health Center Express. Eligible citizens will receive a free rapid diagnostic test for Hepatitis B surface antigen (HBsAg RDT). If the result is positive, the DHC will match eligible participants with family doctors, and participants may choose a Quality HealthCare doctor from the list provided.&nbsp;Please note that, unlike the Chronic Disease Co‑care Scheme (CDCC), registration for the Hepatitis B Co‑care Program cannot be done directly at Quality HealthCare Medical Centers at the moment. Participation is only possible through District Health Centers or District Health Centre Expresses. participants of the Hepatitis B Shared Care Program (Hep. B)&nbsp;are required to pay a one‑off co‑payment of HK$180 to the doctor during the screening stage.&nbsp;During the Treatment Phase, the Government will provide a partial subsidy for each subsidized consultation, while the Scheme Participant will only need to co-pay a fee determined by the doctor upon enrolment to the Scheme. Currently, the co-payment fee charged by Quality HealthCare doctors is $50 per consultation. There is no additional payment on receiving drugs under the list of Specified Drugs and/or up to 3 days of drugs for episodic illnesses.&nbsp;In addition, the Government will provide a partial subsidy for each item of laboratory investigation and each subsidized visit of Nurse Clinic and Allied Health Services. The Scheme Participant will only have to pay the co-payment fee.&nbsp;Eligible elderly person can use Health Care Voucher to settle the co-payment under the CDCC Scheme. The CDCC Scheme has been listed on the ‘Designated Primary Health Care services’&nbsp;(Click <a href="https://www.qhms.com/en/services/public-private-partnership/elderly-health-care-voucher-scheme">here&nbsp;</a>for more details) .&nbsp;Medical fee waiver is not applicable to the Scheme.&nbsp;<h2>What documents do participants need to bring to the consultation?</h2>Scheme Participants who have made an appointment for consultation should bring along their valid Hong Kong Identity Cards or Certificates of Exemption and appointment slips issued by DHC/DHCE (if any) when they visit their doctor's clinics to facilitate identity verification and registration by the clinic staff.&nbsp;Scheme Participants are also encouraged to bring along self-monitoring blood pressure / glucose records and past medical history and medication records for reference by the doctor. The Chronic Disease Co-Care Scheme (The CDCC Scheme) implements the concept of “Family doctor for all” and operates through the interprofessional public-private-partnership model assisted by District Health Centre (DHC)/ District Health Express (DHCE), to subsidize residents to receive diagnosis and management on the chronic diseases that are common in Hong Kong, i.e. diabetes mellistus (DM) and hypertension (HT).

The Chronic Disease Co-Care Scheme implements the concept of “Family doctor for all” and operates through the interprofessional public-private-partnership model assisted by District Health Centre (DHC)/ District Health Express (DHCE), to subsidize residents to receive diagnosis and management on the chronic diseases that are common in Hong Kong, i.e. diabetes mellistus (DM) and hypertension (HT). 

<h3>What is Elderly Health Care Voucher?</h3>Health Care Voucher aimed to subsidised eligible elderly persons to pay for private primary healthcare services.&nbsp;<h3>What is the amount of Elderly Health Care Voucher?</h3>Eligible elderly persons aged 65 or above who hold a valid Hong Kong Identity Card or a valid Certificate of Exemption can receive $2,000 Health Care Voucher annually. Unused voucher amount can be retained for use and accumulated up to $8,000.&nbsp;Eligible elderly persons in spousal relationship can share use health care vouchers between them. &nbsp;<h3>How to use Elderly Health Care Voucher?</h3>Elderly persons are not required to pre-register or collect the vouchers. Health Care Vouchers are issued and used electronically through the eHealth System (Subsidies). The elderly only needs to pick a listed healthcare service providers and use the healthcare services.&nbsp;&nbsp;<h3>How to pair up Health Care Voucher account with spouse?</h3>Prior application is not required. When either party of an elderly couple uses the Voucher to pay for healthcare services, both only need to attend the practice of an Enrolled Service Provider together in person (with HKID card and a Hong Kong phone no. that can receive SMS), to have their Voucher accounts paired up through a one-time registration. Once their accounts are paired up, either party can, upon exhaustion of one’s Voucher account balance, use the remaining Voucher balance in his/her spouse’s account.&nbsp;<h3>How to receive additional Health Care Voucher reward under the Pilot Reward Scheme? &nbsp;</h3>Starting from 13 November 2023, once an elderly person has accumulated Voucher spending of $1,000 or above on designated Primary Health Care services (“PHC services”) within the same year, the eHealth System (Subsidies) will automatically allot $500 reward to the elderly person’s Health Care Voucher account for using on designated PHC services in future.&nbsp;The designated PHC services including:<ul><li>Preventive services&nbsp; (e.g. health assessment, body check, vaccination, etc.)</li><li>Follow-up/monitoring of long-term conditions&nbsp; (e.g. treatment for chronic diseases)</li><li>Chronic Disease Co-care Pilot Scheme&nbsp; (e.g. Hypertension and diabetes mellitus screening services, medical consultations and drugs in treatment phase, etc.)</li><li>District Health Centres/District Health Centre Expresses (e.g. the Chronic Disease Management Programme on osteoarthritic knee pain and low back pain)</li><li>The University of Hong Kong - Shenzhen Hospital and its Health Centre (e.g. Receive outpatient services on preventive and follow-up/monitoring of long-term conditions provided at the designated clinics) &nbsp;</li></ul>Key points:<ul><li>The three-years Pilot Reward Scheme will run up to 31 December 2026. Apart from Medical Practitioners, Chinese Medicine Practitioners, Chiropractors, Registered Nurses and Enrolled Nurses, Physiotherapists, Occupational Therapists, Radiographers, Medical Laboratory Technologists, Optometrists in Part I of the register, the newly- added professionals include Audiologists, Dietitians, Clinical Psychologists and Speech Therapists registered with the relevant healthcare professional bodies accredited under the Accredited Registers Scheme for Healthcare Professions of the Government. Elderly persons can use vouchers to pay for the private primary healthcare services provided by 14 types of healthcare professionals.&nbsp;<h3>How to use the vouchers at Quality Healthcare?&nbsp;</h3>All eligible medical professionals under Quality HealthCare, including general practitioners, specialists, registered Chinese medicine practitioners, physiotherapists, and nurses, are enrolled in the Elderly Health Care Voucher Scheme. It allows eligible elderly individuals to use community care service vouchers to select suitable community care services that support their aging in place under the ‘money-following-the-user’ mode. The scheme covers 18 districts in Hong Kong. Eligible elderly individuals can choose different community care services that suit their individual needs.&nbsp;How much funding is available?Elderly individuals can choose any voucher value to purchase service packages according to their needs. The ceiling monthly voucher values for&nbsp;2026-2027 is HK$10,824 respectively. The SWD will subsidise 60% to 95% of the spending according to the individuals’ affordability, with the remaining amount as the co-payment by the elderly individuals. There are six categories of co-payment. <h3>What is Vaccination Subsidy Scheme?</h3>
Vaccination Subsidy Scheme is a scheme that provides subsidised seasonal influenza and pneumococcal vaccination to eligible Hong Kong residents through the participation of private doctors. The Government reimburses subsidy to enrolled doctors for providing vaccinations to eligible persons. On top of Government's subsidy, enrolled doctors may impose extra charge for the vaccination.&nbsp;
<h3>Why is seasonal influenza vaccination important?</h3>
Influenza occurs in Hong Kong more common in periods from January to March and from July to August. Persons with weakened immunity and elderly persons get infected, it can be a serious illness and may be complicated by bronchitis, pneumonia, encephalopathy, or even death in the most serious cases. Seasonal influenza vaccination is one of the effective means to prevent seasonal influenza and its complications. It takes around 2 weeks after vaccination for antibodies to develop in our body. It is important to get vaccinated early for protection.&nbsp;
<h3>Who can receive free/subsidised seasonal influenza vaccination?</h3>
The 2026/27 Vaccination Subsidy Scheme will commence on 17 September 2026, which&nbsp;covers the following eligible groups&nbsp;(Hong Kong residents) who can receive a government subsidy of $260 per dose for seasonal influenza vaccination:
<ul>
 <li>Pregnant women;</li>
 <li>Persons aged between 50 or above;</li>
 <li>Children and adolescents aged between 6 months and less than 18 years;</li>
 <li>Persons with intellectual disability (PID);</li>
 <li>Persons receiving Disability Allowance;</li>
 <li>Recipients of standard rate of “100% disabled” or “requiring constant attendance” under the Comprehensive Social Security Assistance (CSSA) Scheme; and</li>
 <li>Persons aged 18 years or above but studying at a secondary school in HK (*Please bring along the student card or handbook and submit a photocopy to the doctor)</li>
 <li>Persons aged 18 to 49 years with chronic illness*&nbsp;</li>
</ul>
*Chronic illnesses include:&nbsp;
<ul>
 <li>Chronic cardiovascular (except hypertension without complications), lung, liver or kidney diseases;&nbsp;</li>
 <li>Metabolic diseases including diabetes mellitus or obesity (Body Mass Index 30 or above);&nbsp;</li>
 <li>Immunocompromised states related to weakened immune system due to conditions such as asplenia, Human Immunodeficiency Virus infection/ Acquired Immune Deficiency Syndrome or cancer/ steroid treatment; and&nbsp;</li>
 <li>Chronic neurological conditions that can compromise respiratory functions or the handling of respiratory secretions, or increase the risk of aspiration, or those who lack the ability to take care of themselves</li>

Two doses of influenza vaccines are required for a child aged under 9 years if he/she has not been vaccinated against influenza before.&nbsp;
Remarks: Recipients aged 18 or above must register for eHealth.&nbsp;

<h3>Why is pneumococcal vaccination important?</h3>
Streptococcus pneumoniae (pneumococcus) commonly colonises in the upper respiratory tracts even in healthy carriers. It is the causative agent of pneumococcal infections. The outcomes for invasive pneumococcal diseases are usually more severe among young children and elderly.&nbsp;
<h3>&nbsp;</h3>
<h3>What are the features of the newly registered 15-valent pneumococcal conjugate vaccine (PCV15)?</h3>
There are over 100 serotypes of pneumococcus, of which serotype 3 is one of the serotypes of significant public health concern in recent years. Regardless of age, in recent years, most severe and hospitalised cases of pneumococcal infection are caused by serotype 3. Streptococcus pneumoniae causes a wide range of diseases, including middle ear infection (acute otitis media), pneumonia, and various forms of invasive pneumococcal disease (IPD), such as meningitis and bloodstream infection (bacteremia and sepsis).&nbsp;
 15-Valent Pneumococcal Conjugate Vaccine (VAXNEUVANCE™) is now indicated for active immunization for the prevention of invasive disease caused by Streptococcus pneumoniae serotypes 1, 3, 4, 5, 6A, 6B, 7F, 9V, 14, 18C, 19A, 19F, 22F, 23F and 33F in individuals. VAXNEUVANCE™ specifically targets serotype 3 pneumococcus and helps prevent pneumonia and invasive disease (meningitis, septicaemia, etc.) caused by pneumococcus (including 15 serotypes).
After taking into consideration the local epidemiology and available scientific evidence, the Scientific Committee on Vaccine Preventable Diseases (SCVPD) under the Centre for Health Protection of the Department of Health (DH) recommended to replace PCV13 with PCV15 under the Government Pneumococcal Vaccination Programme. Relevant immunisation schedules for high-risk individuals* would remain unchanged.

Asymptomatic Hong Kong residents can receive screening service in private sector for prevention of colorectal cancer via this programme.&nbsp;Participants will first take a Faecal Occult Blood Test (FOBT) that can detect small amounts of blood in stool, even if they are invisible to the naked eye. The doctor will assess whether they are eligible for joining in the programme. Eligible participants will be issued FOBT tubes to collect stool specimens at home for bringing to designated specimen collection points, which will then be transported to laboratory for analysis. The doctor will notify the test result. The test is fully subsidised by the Government , and hence no co-payment fees will be charged.&nbsp;If the FOBT result is positive, the participant will be referred to see an enrolled colonoscopy specialist to receive a colonoscopy examination subsidised by the Government. Asymptomatic persons aged between 50 and 75, who does not have screening/examination for colorectal cancer within a specified period.

What is Cataract Surgeries Programme? Noting the expanding aging population in Hong Kong, and in view of the steady increase in the number of cataract patients as well as a lengthening of waiting time to undertake cataract surgery in public hospitals over the past few years. Additional funding was earmarked by the Government in 2007 to implement the first ever private-public partnership (PPP) pilot Programme, named Cataract Surgeries Programme (CSP), to provide additional cataract surgeries to meet the growing service demand through a PPP delivery model. CSP Service package Each participating patient will receive one pre-operative assessment, cataract surgery including intraocular lens, and two post-operative checks from the private ophthalmologist that he/she chosen. &nbsp;Eligible person Patient who fulfill the following criteria will be eligible to be invited for the Programme.<ul><li>has been on the HA cataract surgery waiting list for 18 months or above;</li><li>local anaesthesia is suitable for the patient; and</li><li>has been triaged to routine waiting list. (Urgent and early cases need to be handled with priority. Therefore, the HA will continue to arrange surgeries for these cases instead of offering these patients the choice to undertake treatment from private ophthalmologists.)</li></ul>Patients do not need to approach HA. The HA will issue invitation letters in batches to the patients to invite them to apply to participate in the Programme, with priority to those patients who have waited longest on the HA central waiting list. Patient should follow below steps to look for a private ophthalmologist to receive CSP service pacakgePatient should have registered to the Electronic Health Record Sharing System (eHRSS).Patient choose a private ophthalmologist to conduct his/her cataract surgery, he/she should present the his / her Programme Registration Number to the doctor who will then enter this number in the eHRSS to log the subsidy. Once the private ophthalmologist has logged the subsidy, it will be irreversible.The patient must approach private ophthalmologist to receive the pre-operative assessment before the Subsidy Expiry Date and complete the cataract surgery within 3 months after the pre-operative assessment. &nbsp; &nbsp;After completion of the cataract surgery, patient shall sign on the ”Subsidy Redemption Form” to confirm that he has received the cataract surgery.&nbsp;Programme features and service feeCSP provided an alternative for eligible cataract patients to undertake cataract surgeries in the private healthcare sector, and helped other cataract patients indirectly by shortening the waiting list and notional waiting time in HA.For participating patients selected Quality Healthcare’s ophthalmologist, patient need to pay HK$8,000 as co-payment and patient will receive a fixed amount of HK$8,000 subsidy for the service package.Those who are Comprehensive Social Security Assistance Recipient, certificate holders for full or partial waiver of medical charges or Old Age Living Allowance Recipient aged 75 or above will enjoy the same fee waiver arrangements as for the HA’s services.* Elder Healthcare Voucher is not applicable to the co-payment of CSP service package&nbsp;How to receive CSP service package from Quality HealthCare? Patients can easily access primary care services in a efficient way.&nbsp;&nbsp;<h3>How to receive private healthcare service at the fee of the General Outpatient ?</h3>Each participating patient will receive up to 10 subsidised visits per year, including medical consultations covering both chronic and episodic illnesses; drugs for treating their chronic conditions and episodic illnesses to be received directly from private doctors at their clinics; and investigation services provided by the Hospital Authority as specified through private doctors’ referral.&nbsp; &nbsp;&nbsp;Under the Programme, participating patients only need to pay the fee of the General Outpatient Clinic under the Hospital Authority at $50 for each consultation.&nbsp;&nbsp;Participating patient who is eligible for a waiver under the criteria set out in the Notice on Waiver Arrangement in Programme’s website may enjoy the same fee waiver arrangement as for the Hospital Authority’s service when he/she attends the private doctors for services under the scope of the Programme.&nbsp;<h3>How to use the services of Quality Healthcare?</h3>Over 60 general practitioners under Quality HealthCare have enrolled in this programme. <figure class="image image-style-align-center"><img src="/uploads/image_903f3161f6.png" alt="Logo for General Outpatient Clinic Public-Private Partnership Programme"></figure>&nbsp;<h3>甚麼是「普通科門診公私營協作計劃」?</h3>長期在普通科門診覆診、患有高血壓及/或糖尿病（或附帶高血脂症）而病情穩定的病人，可透過醫院管理局「普通科門診公私營協作計劃」，接受社區內的私營基層醫療服務，讓病人方便快捷地使用基層醫療服務。&nbsp;<h3>如何以普通科門診服務收費，使用私營醫療服務?</h3>參加計劃的病人每年可享有多達10次資助門診服務，包括診治慢性疾病和偶發性疾病、於私家醫生診所獲配治理其慢性疾病病情及偶發性疾病的藥物、以及經參加計劃的私家醫生轉介，接受由醫院管理局提供特定的相關化驗和X光檢查服務。&nbsp;病人每次就診只須支付普通科門診服務的收費50元。符合資格獲得豁免的計劃病人，向本計劃的私家醫生求診時，若屬本計劃範圍內，可獲得與使用醫院管理局服務相同的費用減免安排。&nbsp;<h3>如何使用卓健醫療的服務?</h3>卓健醫療旗下近60位普通科醫生均參與計劃，並開展「普通科門診藥物處方優化計劃」，在現行計劃的特定藥物名單以外，增備19種藥物(調節血脂、降血糖藥物、抗生素、外用抗真菌藥及抗炎止痛藥等)，期望提供更多適切的藥物選擇，令基層醫療服務更便捷和靈活，卓健醫療旗下診所只收取醫院管理局普通科門診服務費用50元。&nbsp;<a href="https://www4.ha.org.hk/ppp/ppp-programmes/gopcppp/programme-intro" target="_blank" rel="noopener noreferrer">醫院管理局「普通科門診公私營協作計劃」</a>
